Mend will be hosting Drive-Thru COVID-19 Testing dates Monday through Friday 9a-5p at the following locations:

PLEASE PRE-REGISTER ONLINE AND RECEIVE AN APPOINTMENT TIME TO BE ACCEPTED FOR DRIVE-THRU TESTING. INDIVIDUALS WITHOUT REGISTRATION WILL NOT BE ALLOWED ON SITE. PLEASE ONLY ARRIVE AT THE APPOINTMENT TIME ON YOUR E-MAIL CONFIRMATION. PLEASE READ BELOW DETAILS BEFORE REGISTERING.
NOTE: Patients must meet current CDC Guidelines and Los Angeles County Department of Public Health criteria to qualify for COVID-19 testing, and must be determined by our clinical providers to be appropriate for testing.
NOTE: Under the Families First Coronavirus Response Act (“The Coronavirus Act”), it is ordered that all COVID-19 testing is covered by health insurance. In accordance with The Coronavirus Act, the in vitro diagnostic testing performed by Quest Diagnostics (the laboratory) and medical examination related to the furnishing or administration of this test by Mend Health, Inc. (the provider) are considered covered services by Section 6001(a) of the Coronavirus Act and as such, medical claims will be sent to my insurance for these services.
NOTE: Quest Diagnostics (the laboratory) will bill my insurance for COVID-19 testing and will attempt to collect payment. If my insurance company does not pay, I will be responsible for the laboratory fee issued by Quest for the testing. In this event, I can submit this bill denied by my insurance company for reimbursement. NOTE: Mend Health, Inc. (the provider) is not associated with any of the laboratory processing fees, and any disputes on laboratory processing fees must be handled directly with Quest Diagnostics and your insurance plan.
